Shakes or Wobbles problem

2003 Chrysler Town and Country  6 cyl Two Wheel Drive Automatic 100000 miles



When I apply the brakes on my van while moving there in a jerk after several seconds, not immediately upon applying the brakes. The jerk almost seems as if the van is shifting to a lower gear, it is not. After the jerk, the brakes work normally. A brake job was done on this vehicle within the last 3000 miles. Brakes or transmission? All help greatly appreciated.
