Transmission problem

2001 Chrysler Town and Country  6 cyl Front Wheel Drive Automatic 158000 miles



I had the transmission rebuilt about 30k milles ago and only recently has it been failing to shift to 3rd or 4th gear when warm.  It also has some shifting issues when shifting from 1st to 2nd it will sometimes shift back into first for no apparent reason.  It had started to send check engine code but one of the mechanics cleared the code and replaced a fuse and the codes are no longer showing. Since this began, I had the soliniod pack replaced with no improvement.  What other possible cause am I looking at.  The dealer changed the fluid on my last oil change so I have clean fluid and cannot check for color or smell.
